Kalamazoo County Community Health Worker Training Grants
Opportunity Overview
- Programs operated by 501c(3) organizations are eligible to apply for a portion of $45,000 of funding to support the training of Community Health Workers in Medicaid billing procedures.
- Minimum request amount of $5,000.00
- One year grant term (July 1, 2024-June 30, 2025).
- Payment of the full annual amount awarded will occur at the beginning of the grant year (July 2024).
- Direct administrative costs can be included in the request. No indirect administrative cost can be charged. Definition of Direct vs Indirect Administrative Costs
- Quarterly reporting reconciling budget and outcome data, including expenditure documentation, required.
- Dedicated UWSCMI staff liaison will work with funded partners throughout the grant term to support success in meeting all grant requirements.
Eligibility Criteria
- Active 501c3 status
- Application completed and all federally required documents provided (UEI number, proof of insurance, financial documents, code of ethics/conflict of interest, record retention policy).
- Organization is not suspended or barred from receiving federal funding or operating federal program(s).
- Program implements training for Community Health Workers to learn Medicaid billing procedures to increase program sustainability.
- Program serves residents in Kalamazoo County.
Notes:
- If financial criteria are not able to be met, an organization may ask another organization to serve as fiscal sponsor.
- Priority may be given to proposals with documented collaboration between two or more community organizations.
- Applicants are strongly encouraged to meet with UWSCMI staff prior to submitting an application. Make an appointment.
Eligible Activities
Each funded project must align with specific eligible activities under SLFRF (State and Local Fiscal Recovery Funds) Expenditure Category 1.11 – Community Violence Intervention. As part of the application, applicants will be asked to identify the action step below that most aligns with their work:
- Provide training for Community Health Workers to bill Medicaid for relevant violence intervention work.
Process Timeline
March 18, 2024
- Requests for proposals released.
- UWSCMI staff, County Commissioners, and community partners conduct targeted outreach to eligible grantees.

March 18-May 3, 2024
- Applicants work to complete application. Ongoing technical assistance from United Way staff available.
May 3, 2024, at Noon
- Application Submission Deadline
May 6-24, 2024
- Staff/Community Consultants review applications.
June 4, 2024
- Grantees notified of outcome.
- Grant Agreements are provided to approved grantees. Those receiving partial funding are given the opportunity to adjust projected outcomes/budget.
June 15, 2024, at Noon
- Grant Agreement Submission Deadline
Week of June 18, 2024
- Verification of all grant requirements. Grant Agreements signed by UWSCMI and returned.
July 1, 2024
- Grant term begins.
- Ongoing UWSCMI staff support provided for reporting and compliance with federal funding requirements throughout the grant term.
Ongoing through grant term
- Quarterly reports submitted on July 10th, October 10th, and January 10th, and April 10th of the grant year.
June 30, 2025
- Grant term ends. Final report submitted on July 10, 2025.
